For thousands of years, everything Apocalypse did was preparation for one reunion. His wife Genesis and his four children, the first Horsemen, had sealed themselves inside the hell-dimension Amenth with the mutant island Arakko, holding back its demon armies while he stayed on Earth to forge a mutant society strong enough to survive their return — a history laid out in X-Men Vol. 5 #12 (2020).
When the two sides finally met in Otherworld, Genesis was wearing the Golden Helm of Annihilation, the entity that had ruled Amenth through her. Apocalypse fought his own wife, tore the helm free, and let it take him instead — then did the one thing nobody expected from the prophet of survival of the fittest: he admitted he was the weaker of the two, surrendered, and ordered the Amenthi horde to stand down with him.
Peace came with a prisoner exchange. Genesis, speaking for Arakko, chose a single prisoner to take back to Amenth: her husband. Apocalypse, speaking for Krakoa, chose his in return — the entire island of Arakko and its millions of mutants, freed from Amenth at a stroke. He then left Earth with his family, vacating the Quiet Council seat he had accepted in House of X #5 (2019).
