When Johann Schmidt's original body finally gave out under its own age, death did not get to keep him. Arnim Zola had preserved the Skull's consciousness, and in the back story of Captain America vol. 1 #350 (1989) that mind was poured into a fresh body grown from DNA taken from Steve Rogers while Rogers was the Skull's prisoner.
The clone carried the Super-Soldier Serum in its veins, so Schmidt woke up with Captain America's strength, speed and stamina — and, at first, Captain America's face. For a stretch of stories the most hateful man in the Marvel Universe could walk through a crowd wearing the features of its greatest hero, and nobody looking at him could tell the difference.
The disguise ended in that same issue's three-way fight between Rogers, his replacement John Walker, and the Skull. Schmidt was blasted with his own signature weapon, the Dust of Death — and instead of killing the serum-charged clone outright, the dust shrivelled its face into a permanent red skull, branding the borrowed body with the visage he had once only worn as a mask.
Why It's Obscure
He did not just imitate his enemy this time — he moved into a copy of him, and his own poison evicted the disguise.
